Oriented Times One

I am rounding on Gyn with the 5th year med students.
A 24yo P1 HIV+ with psychosis and possibly PID or cervical cancer, refusing exam.
I try to assess her mental status.

Hello, how are you?
I am fine, thank you.

Are you in pain?
No pain, I am fine, thank you.
Do you know where you are?
I am here!
Where is here?
Is this home?
No. Home is there!
So where are we?
In the store?
In the hospital?
Do you know what year it is?
Do you know who the preseident is?
President Museveni.
But he is dead.
[He’s not.]
He is?
Yes. He fell in the river.
[He didn’t.]
Do you know who I am?
Who am I?
You are mzungu!

I had to give her that one.
